Transcript Request Form
What is an official transcript?
Transcripts contain confidential information, and they are only released in accordance with the Family Educational Rights and Privacy Act of 1974.
A transcript is a comprehensive record of your academic work. It will include courses transferred into ASU, credit earned by exam, and any degrees you have been awarded by Angelo State University. Each student's ACT and/or SAT scores (if they were reported to ASU) and TASP (Texas Academic Skills Program) scores are printed on the transcript. Each transcript is printed on security paper to prevent copying, and the document includes the seal of the university and the signature of the University Registrar.
To request an official transcript, please follow the instructions below. You MUST complete and mail a paper copy of the request form below to request a transcript. E-mail and telephone requests will not be honored for security reasons. Each request must contain an original signature of the student whose transcript is being requested. Your understanding of these requirements is appreciated.
You may request a transcript in person by coming to the Office of the Registrar on the ASU campus, Hardeman Building Room 101 and completing the Transcript Request Form. Copies of transcripts for personal use follow the same policies as those used for transfer of information to other institutions, employers or prospective employers, or other entities requiring student academic information.
Cost
There is no cost for the first 20 transcripts requested each academic year per student, including former students. Each student is assessed a Records Maintenance Fee of $10.00 each semester which covers, among other items, the cost of processing transcripts. Each transcript beyond the first 20 will be assessed a $3.00 fee for current students and former students.
Turn-around time
Transcripts are normally processed within one business day of receipt. During peak times, such as registration and/or graduation, please allow additional time for the processing of your request. The Office of the Registrar at Angelo State University will not honor a request for express mail or courier service of transcripts. Students requiring express mail or courier service may pick up the sealed copy of their official transcript and make their own arrangements for such service.
Holds
If there is a hold on your record, which prevents you from receiving a transcript, you will be notified in writing that your transcript was not sent due to a hold on your record. The office, which placed the hold, will be listed on the receipt. You must contact the office listed to have the hold removed.
INSTRUCTIONS FOR COMPLETING THE TRANSCRIPT REQUEST FORM
- Print the form.
- Fill in your full name and current mailing address in the space available at the top of the form.
- In the box labeled "FOR COMPLETION BY STUDENT " fill in the blanks to indicate the number of copies desired, if you wish for the Registrar's Office to hold the transcript, and the purpose of the transcript.
- In the space at the bottom-left of the form, indicate the address to which you want the transcript sent.
- In the space at the bottom-right of the form, complete the information requested (SSN, name, name under which enrolled, date last enrolled, graduation date).
- SIGN THE FORM; without an original signature, the request will not be processed; electronic, or scanned signatures are NOT valid for requesting a transcript.
