Applying for Financial Aid 2007-2008 & 2008-2009

Free Application for Federal Student Aid (FAFSA)
Priority Deadline: Apr. 1
Submit on-line the FAFSA or renewal FAFSA to apply for federal and state grants, loans, need-based scholarships, and Work-Study. Include ASU’s Federal School Code (#003541) in the School Section to ensure electronic access to the Student Aid Report (SAR) information by the Financial Aid Office. Allow approximately 2 weeks processing time for electronic submission and processing. Additional documents may be required if your FAFSA information (SAR) is selected for the process of verification or to clarify your individual circumstance. You will be notified if additional documents are required.

General University Scholarship Application 2008-2009
Priority Deadline: Mar. 1
Submit this on-line application to apply for the various scholarships awarded by the Financial Aid Office (excluding Carr Academic Scholarship).

Summer Supplemental Application 2008-2009
Priority Deadline: March 1, 2009
Submit this on-line application to apply for financial aid for summer I & summer II (excluding Carr Academic Scholarship)

Regent Kinship Statement 2008-2009
Submit this on-line certification of kinship to a TTUS Board of Regent.

Disclosure of Additional Resources: 2008-2009
Submit on-line form if you expect to receive additional assistance from outside sources during the application period.

The following forms are not available to be transmitted on-line but may be completed and/or printed for mailing/faxing to our office.

Dependency Change Request: 2008-2009
These forms are used by students who do not meet the definition of an independent student as outlined in the Free Application for Federal Student Aid (FAFSA) and would like to apply for a dependency change due to their unusual circumstance.

Dependency Change Request - Renewal: 2008-2009
This form is used by students who have been approved for a dependency change in the previous year and would like to renew the request for the next application period.

Special Circumstance Application: 2008-2009
This form is used to document a significant change for the worse in the financial situation of the student, the student's spouse, and/or the student's parent(s). Changes may include a loss of job or benefits, extreme medical expenses, etc..

Verification Worksheet: 2008-2009
This form is used by students who have been chosen for verification by the federal processor when their Free Application for Federal Student Aid (FAFSA) was processed. (Do not submit this form unless it is requested by the Financial Aid Office.)

Statement of Income and Expenses: 2008-2009
Due to the minimal amount of income reported on the financial aid application, this form may be required for additional clarification of the student's/parent's situation.
(Do not submit this form unless it is requested by the Financial Aid Office.)

Federal Parent Loan (PLUS) Borrower Authorization Form:
After the student receives the award notification and enters on-line acceptance for the PLUS award, it is required that the parent borrower complete this form and submit it to the Financial Aid Office.

Emergency Loan Application and Promissory Note
This form is used to apply for short-term university loans in anticipation of financial aid awards or for those students who need help in paying for tuition and fees and are not applying for financial aid. These loans are due and payable within the semester they are borrowed.

Perkins Loan Master Promissory Note
This form is used to finalize your awarded Perkins Loan. Funds will not be disbursed until the completed promissory note is returned to the Financial Aid Office. Faxes cannot be accepted as we must have the original signed document. Please do not complete this promissory note unless you have been awarded a Perkins Loan.

Hinson-Hazlewood College Student Loan Program (Exit Interview)
Students that borrow funds from the College Access Loan program (CAL) from The Higher Education Coordinating Board and drop below half time or graduate you would need to complete the Exit Interview. Completed forms must be returned to the Financial Aid Office. (Do not submit this form unless it is requested by the Financial Aid Office.) Completed forms can be dropped of at our office Monday-Friday, by fax 325/942-2082 or by mail to:
